The History and Physical Exam, often called the "H&P" is the starting point of the patient's "story" as to why they sought medical attention or are now receiving medical attention. The Physical Exam includes both objective and subjective assessments of the patient's physical being.

A Medical Records Release Form is used to request that a health care provider (physician, dentist, hospital, chiropractor, psychiatrist, etc.) release a patient's medical records, either to the patient, a third party (such as an employer or insurance company), or both.

This form is for use when such authorization is required and complies with the Health Insurance. Portability and Accountability Act of 1996 (HIPAA) Privacy Standards.
